Desert Spine and Scoliosis Center of Arizona is not only a venue for top orthopedic spine surgery but also a place for superb patient care and convenience. For this reason, we are constantly seeking to facilitate our patients’ experience in every way.
Information about Requesting Medical Records
A patient’s privacy is protected by law. The law only permits disclosure of medical information if the patient or their legal representative consents to such a release in writing. In other words, in order for our center to release your information to a different physician, health care provider or third party, you will have to consent to this by completing a Release of Medical Information Authorization Form.
This release may be required for several reasons, including:
Sharing of information between your healthcare providers.
- A job application.
- Applications for certain health or life insurance policies.
- Sharing of information with attorneys in the event of a personal injury claim.
Other than the patient, who else can authorize release of records?
Generally speaking, only the patient can authorize release of their own information, however, in the case of minors, a parent or legal guardian may provide this authorization to consent to the release of records. A person who has power of attorney may also authorize release of medical information.

Upon receiving your request, we will make every reasonable effort to compile and forward your medical records/x-ray requests in a timely manner. Please understand that in rare cases, particularly if we have not seen you for some time or if your requested records have been stored off-site, your request may take as long as two weeks to complete.
